Attention!

This list of documents sufficient to make a passport with our help:

• Passport of citizen of Ukraine;
• Identification number;
• For men to 25 years - from military registration certificate;
• Women who changed the name of marriage - an identification code to a new name (same as in civilian passport);
• Old passport (if available).

Documents for children’s travel passport:

• Parents permission notarized by a notary for the travel documents for the child;
• Birth certificate of child;
• 4 color matte photos (3.5 x 4.5 cm).

Children to 17 travel abroad with children’s travel document.

If parents are traveling with a child which no child travel document, information about such child must be in passport of one parent.

If a child less than 5 years in parents’ passport should be affixed her photo

If the child will travel with only one of the parents needed original notarized authorization from the other parent.

Documents required for entering information about a child in the passport of parents:

•     Identity passport of parents;
•     Foreign passport of parents;
•     Birth certificate of child;
•     2 matte photos (3.5 x 4.5 cm)
•     Non-urgent recording, Gluing 200 UAH.
•     Urgent recording, Gluing 300 UAH.
